The combination of dental implants to support the RPD might alleviate some of the problems associated with conventional RPDs and also offers an effective treatment alternative to patients where implants supported prostheses is not feasible or desirable because of emotional or financial limitations.

Studies have reported that the use of implants in conjunction with remaining natural teeth in the implant supported RPD substantially improve oral health quality of life for patients compared to conventional treatment options. The patients receiving this treatment had only minor prosthetic complications, great satisfaction, and improved chewing efficiency. This modality can be a rewarding treatment option as it can provide outstanding comfort and satisfaction to patient and offers prosthetic simplicity and the ability to perform maintenance and modification over the long term to clinician.

Implant selection and positioning can be confidently addressed with thorough, prosthetically driven treatment planning. The appropriate size and spacing of dental implants in the posterior is determined by carefully assessing the anatomy of the ridge, the restorative space available, and the long-term needs of the patient.
The edentulous span must be evaluated to identify the appropriate implant size for each site. First, the mesial-distal distance between the teeth adjacent to the edentulous space is measured. Then, the implant sizes are determined, allowing for 1.5 mm of space between the shoulder of each implant and the neighboring tooth, and 3 mm between adjacent implants. Buccolingually, allow for 1–2 mm of bone on each side of the implant.

Fabricating zirconia crowns using a digital workflow combines precision technology with modern ceramic materials to create highly accurate, esthetic, and durable restorations. The process begins with an intraoral scan that captures detailed 3D images of the prepared tooth, eliminating the need for traditional impressions. These digital scans are then used in CAD software to design the crown, ensuring optimal occlusion, contours, and fit. The design is sent to a CAM milling machine, which carves the crown from a pre-sintered zirconia block. After milling, the crown undergoes high-temperature sintering to achieve its final strength and dimensions, followed by staining and glazing for natural esthetics. The final restoration offers excellent marginal integrity, biocompatibility, and long-term durability, while the digital workflow streamlines communication between clinic and lab, reduces chair time, and enhances patient comfort.

In a full mouth reconstruction, temporary restorations made of PMMA (polymethyl methacrylate) are often used to allow patients to function and smile while implants or other permanent restorations are being fabricated. It will also allow a testing period for the patient to assess comfort and function. PMMA is a type of acrylic that is strong, lightweight, and can be made to look natural, providing an aesthetically pleasing temporary solution. These temporary restorations are crucial for maintaining function and aesthetics during the healing process after procedures like dental implant placement or surgical crown lengthening.

PMMA can be fabricated in a dental laboratory or even in the dental office, and different curing methods (heat, cold, light) can be used. Once the function and aesthetics of the new design is approved, a copy of the provisional restorations can be made and shared with the lab for fabrication of final ceramic restorations. Usually the provisional restorations are is sections with attached contact area, as compared to the individual ceramic restorations.

As implant therapy evolves, new challenges are faced as result of higher functional and esthetics demands. Original protocols during the late 1970s and 1980s advocated the placement of dental implants exclusively in completely healed edentulous ridges. Additionally, a healing period of 6–12 months before loading was recommended.

Fortunately, as a consequence of continuous research, new surface technologies, and new implant designs, more recent protocols have arisen shortening the time intervals by means of immediate implant placement (IIP) and/or immediate restorations (IR).

Prosthetically driven implant placement prioritizes the desired final prosthetic outcome when planning and executing dental implant surgery. Patient who received this dental treatment is a female and 67 years old. Her upper anterior teeth were restored with a combination of composite veneers, a metal framework resin-bonded bridge (Maryland bridge) to replace #1.2, and class III and IV composite restorations. Periodontal bone loss and gingival recession with 2-3mm root exposures. Templates were made based on a digitally designed smile and anatomy for upper teeth in esthetic zone, followed by removal of old restorations, core build ups and interim restorations in acrylic material. Patient functioned with the interim restorations for a few weeks and once aesthetics, phonetics and chewing efficiency were confirmed, final monolithic zirconia restorations were fabricated, tried in and delivered to the patient. Pictures are from a 1 month follow-up appointment. An occlusal appliance was fabricated and delivered to be used on a regular basis.

The provision of implants following traumatic dental injuries can hold many challenges, primarily in higher aesthetic regions. The two central incisors were removed atraumatically and NobelReplace Tapered TiUltra implants were placed into the fresh socket and PRF + Xenograft bone biomaterial were used to augment the extraction socket. 8 years follow up.

Full mouth reconstruction refers to rebuilding and/or replacing all of the teeth in a patient’s mouth. Full mouth reconstructions combine esthetics with the science of restorative dentistry to improve the health, function, and beauty of the mouth.

There are a number of people who have dental problems throughout their mouths that must be treated comprehensively with a vision of a final result that improves both function and esthetics. Patient who received this dental treatment is a female and 57 years old. Her upper anterior area was damaged in a car accident. Teeth 1.2 and 2.2 were fractured horizontally and could not be restored. Teeth 1.1 and 2.1 had no fractures and were managed with an occlusal stabilizing device. 1.2 and 2.2 were replaced with implant crowns: extraction, immediate implant placement and fixed temporary crowns: @nobelbiocarecanada #NobelActive TiUltra NP and final restorations with #NobelProcera custom abutments and layered zirconia crowns. Bridge 1.3-1.5 was also replaced to address the marginal defects and recurrent caries.

Restoration of lower anterior teeth with dental implants is one of the challenging treatments. Extraction socket was grafted with a mix of PRF and xenograft bone biomaterial and covered with PRF and dPTFE membrane. Healing time before second stage surgery: 4 months. Patient had recently received a full mouth reconstruction at a GP office in lower mainland, BC. Her chief complaint was food impaction and continuous pain at the gums, specially in quad 1 and 4.

Initial radiograph:
- Open crown margins
- Inadequate endo treatment
- Caries at Bu and Li furcations

Diagnosis: after removal of existing crowns and assessment of remaining tooth structures following treatments were recommended:

Zirconia restorations, manufactured exclusively by CAD/CAM technology, have considerable advantages: they exhibit high flexural strength, require more conservative dental preparation, minimize wear on the antagonists, exhibit satisfactory aesthetics, require less laboratory time and fewer dental sessions, and as monolithic, they lack the unwanted complication of chipping. Their main disadvantage until a few years ago was their low aesthetic performance due to the inability to achieve satisfactory transparency. However, recent modifications in composition, structure, and fabrication methods have led to monolithic zirconia ceramics of superior translucency, but with a significant reduction in strength.

Full mouth reconstruction
There are a number of people who have dental problems throughout their mouths that must be treated comprehensively with a vision of a final result that improves both function and esthetics. These patients may exhibit multiple missing teeth, numerous teeth with large fillings that are failing or exhibiting decay, cracked or broken teeth, or badly worn teeth due to bruxism (teeth grinding) or other habits.

There is also a group of patients who were born with conditions such as Ectodermal Dysplasia, Ameliogenesis, or Dentinogenisis Imperfecta that will need extensive restoration of their teeth. These patients may be candidates for a full mouth reconstruction.

Immediate implant placement is defined as a placement of a dental implant at the time of tooth extraction, into the extraction socket. This has been claimed to shorten treatment time due to reduction the number of surgical procedures, maintain soft tissues, and possibly preserve surrounding bone tissue and guide for implant placement.

There are a number of similarities between implant placement in a healed site and immediately after tooth extraction: management of hard and soft tissues, implant stability, three-dimensional positioning, prosthetic placement, implant size and connection choice, the list can go on and on. However, there are several key differences between immediate and healed implant site placement. Immediate implant placement requires a deep knowledge of four crucial aspects present at the time of the intervention. These aspects are: management of the extraction site, anticipation of the healing of the socket, the position of the implant relative to the available and anticipated hard and soft tissue positions, and finally the stability of the implant in the correct prosthetic positions.

Internal root resorption (IRR) is a particular category of pulp disease characterized by the loss of dentine as a result of the action of clastic cells stimulated by pulpal inflammation. One of the indications for tooth extraction is a root resorption that cannot be treated endodontically.

The case presented here shows a dental restoration with the extraction of teeth 11 & 21 due to significant internal root resorption and immediate implant placement and soft tissue augmentation with a connective tissue graft.

The direct sinus lift procedure has been in use for decades and is a reliable way to increase bone volume in the posterior maxilla. Surgeons have many grafting materials available for use including autogenous bone, allograft, xenograft, and bone morphogenic protein. Contemporary modifications to the direct sinus lift include modern instruments, for example, the piezoelectric, and technological advancements such as computer-aided implant planning software to create a fully digital workflow. The direct sinus lift remains a reliable strategy when a significant amount of bone gain is required in the posterior maxilla with implant survival typically greater than 95%.

The placement of a dental implant in an extraction socket at the time of extraction or explantation is known as immediate implant placement. Recent idea goes by “why late when it can be done immediately.” There are several advantages of immediate placement of implants. Case selection, proper diagnosis, treatment planning, and initial stability are very important factors for the success of an immediate implant.

The management and prevention of soft tissue complications is of key importance in modern implant dentistry and influences biologic and esthetic outcomes. Female 45 years, ASA I, Class II jaw relationship. Missing first premolars. Teeth 1.5 and 1.6 were non-restorable and were replaced with immediate implants. Anterior teeth were damages in an accident a few years ago and were restored with a single crown and composite veneers. Patient wanted a natural and fresh smile design. Natural teeth are restored with a combination of full coverage crowns and porcelain veneers made with layered Ivoclar e.max. Implant crowns are monolithic Zirconia over Titanium bases.
